Liability Waiver — Sagrado San Pedro Church

Liability Waiver & Release Sagrado San Pedro Church — General Membership

Please read this document carefully and in its entirety before signing. This is a legally binding agreement.

1. Nature of the Church and Its Activities

I understand that Sagrado San Pedro Church is a religious community that conducts sacramental worship services in which Huachuma — Trichocereus pachanoi — is administered as a consecrated sacrament. I understand that Huachuma contains mescaline, a naturally occurring psychoactive compound. I am entering this community with full knowledge of the nature of its sacramental practice.

2. Voluntary Participation

I affirm that my membership in Sagrado San Pedro Church and my participation in any and all Church activities is entirely voluntary. I have not been coerced, pressured, or misled. I am joining of my own free will and informed choice.

3. Personal Responsibility

I accept full personal responsibility for my participation in all Church activities, including sacramental worship services. I affirm that I am in adequate physical and mental health to participate in the Church's activities. I understand it is my responsibility to disclose any health conditions, medications, or circumstances that may be relevant to my participation, and to consult a qualified healthcare provider before participating in any sacramental worship service.

4. Assumption of Risk

I understand and acknowledge that participation in sacramental worship services involving Huachuma carries inherent risks, including but not limited to: physical discomfort, altered perception, emotional intensity, and unpredictable individual responses. I voluntarily assume all such risks.

5. Release of Liability

In consideration of being accepted as a member of Sagrado San Pedro Church, I hereby release, waive, and discharge Sagrado San Pedro Church, its Board of Directors, Officers, Ministers, Sacramental Officers, agents, and volunteers from any and all claims, demands, damages, losses, or liability arising out of or in connection with my membership or participation in any Church activity, to the fullest extent permitted by law.

6. Indemnification

I agree to indemnify and hold harmless Sagrado San Pedro Church and its Directors, Officers, and Ministers from any claims brought by third parties arising out of my participation in Church activities.

7. Governing Law

This waiver shall be governed by the laws of the State of California.

8. Acknowledgment

I have read this waiver in full. I understand its contents. I sign it freely and voluntarily.
